HIPAA - Notice of Privacy Practices

This Notice of Privacy Practices describes how your protected health information (“PHI”) may be used and disclosed, and how you can access this information. We are required by law to maintain the privacy of your PHI under the Health Insurance Portability and Accountability Act (HIPAA).

We may use or disclose your information for:

• Evaluation & treatment purposes

• Consultation and coordination with other providers (with your authorization unless otherwise permitted by law)

• Billing and administrative purposes

• Required legal or regulatory reporting

2. Uses & Disclosures Requiring Authorization

We must obtain your written authorization for:

• Release of full psychological or psychoeducational evaluations

• Sharing PHI for any reason not covered under this Notice

• Marketing, testimonials, or external case discussions

You may revoke authorization at any time in writing.

3. Your HIPAA Rights

You have the right to:

• Access your record

• Request corrections

• Request confidential communication

• Request restrictions

• Receive a list of disclosures

• Obtain a paper copy of this Notice at any time

Requests can be submitted in writing to our office.

4. Our Responsibilities

We must:

• Protect your information

• Notify you in the event of a breach

• Provide you with updated Notices of Privacy Practices

• Follow all confidentiality laws

5. Questions or Complaints

If you believe your privacy rights were violated, you may file a complaint with us or with the U.S. Department of Health and Human Services.
